Post high school transition planning starts at age 14 in Wisconsin. It is a collaborative effort that involves families, students, school and other agencies as needed. Below you'll find links to a small sample of resources that are available on the Wisconsin Transition Improvement Grant web site. Followed by a link to the Department of Public Instruction's Transition web page and downloadable versions of their Opening Doors series of publications.
